Families who wish to complete their survey must carry the following documents with them while going to complete the survey:.
- ID card
- Children’s Bay Form
- Household electricity bill
- Household gas bill
- Proof of monthly income
- Proof of monthly household expenses
- Disability certificate for disabled persons
- Special identity card for transgenders
- Death certificate of husband for widows

Process after completing the survey
As soon as a family completes its survey, the verification process by BISP begins. During the verification process, the BISP staff thoroughly checks the information submitted by the concerned family. After completing the screening process, the BISP staff assigns a poverty score based on the financial and social status of the family concerned. If the score is below 32, the family becomes part of the BISP Kafalat program. Being a part of the Kafalat program, financial assistance is provided to such families on a monthly basis every three months.
